​Jaqueline Chacon, FNP, APRN

Jaqueline was born in Los Angeles, California with her mom, dad, and two brothers, as well as the rest of her maternal grandparents, uncles, aunties, cousins, and extended familia. She then moved to El Paso, Texas when she was 5 and made it her home. She met her high school sweetheart in El Paso and they have been married for almost 23 years now.  Her husband is a service-connected retired veteran of the United States and was stationed here on the island of Oahu for his last tour of duty in 2004. They fell in love with Hawaii, its culture, the aloha, the oceans, lands, and most of all, the people. 

After high school, Jacqueline got her certified nursing assistant (CNA) certificate and worked throughout El Paso in long-term care facilities, rehabilitation facilities and medical hospitals. 

She then started college at the El Paso Community College for her basic credits for nursing. She wanted to follow in her mom and family's footsteps since she has a family of nurses. 

After obtaining her basic credits, she started the Registered Nurse program at the University of Texas at El Paso for her RN BS - "Home of the mascot the Miners!" 

From 2000-2010, she had the pleasure of working as a RN throughout Nebraska due to it being her husband's first duty station for about 3 years. She worked at the Midlands Community Hospital on their ICU/ telemetry/ medical surgical floors.

She then was asked to work for a grant-funded medical program with the University of Nebraska, where she helped immigrant workers with health screening, examinations, as well as classes on wellness.  She even assisted with health care to only Spanish-speaking women and children. It was here where she was able to present mass safety education and high blood pressure / diabetes / high cholesterol screening and education to beef packing facilities and in the town of south Omaha. 

Shortly after a 911 tragedy occurred and her husband went off to war, she went back home to work for Providence Memorial Hospital in their renal/medical surgery/telemetry ward for one year.

They then came to Hawaii in 2004 and started to work for Tripler Memorial Hospital on their telemetry/Med surf unit for 3 years then transferred to the Spark M. Matsunaga VA Medical Center, when she decided to continue her education again in 2007.

She started her MSN at Hawaii Pacific University and graduated with her Advanced Practice Registered Nurse (APRN) in 2010. 

She has worked throughout Oahu since then with various clinics in family medicine since then.

Mission 

Jacqueline's mission is to treat everyone with compassion and respect and to teach people about their bodies and how to care for it. She also aims to educate others on disease prevention and health promotion, as well as to educate those with chronic illness on how to best manage their disease to promote better quality of life.
